Patients who test positive for HIV are required to undergo CD4 test routinely as part of follow up. Patients with a weak immune system will have very low levels of CD4 cells. This is because the immune system cannot produce adequate CD4 cells to protect the various body tissues. Some times cancer patients too develop lowered CD4 count if they are taking chemotherapy for a long time. Any infectious or harmful influence or medical treatment which will suppress activity of the patient's immune system will lead to lowered CD4 levels.

A venous blood sample is withdrawn from a vein in the forearm to perform this test. Blood sample is stored in a sterile and dry container which already contains anti clotting medicines. The sample is stored away from heat and light till it is tested. Normal levels of CD4 cells in a healthy adult are 500-6000/ mm3. Any levels below 500 or above 6000 require further investigation.

Patients may also require to undergo CBC, ESR, CRP and other blood tests along with CD4 test. Depending on the underlying disorder, patients may require to undergo scans like USG, MRI and CT scan. CD4 is an extremely sensitive test and results get affected by factors such as physical stress, time of the day when the test was performed, medication etc. Therefore, patients who need to undergo this test regularly are advised to take the test only after consulting with a doctor.

Q: What are CD4 cells?
A: Also known as T-helper cells or CD4 T lymphocytes, they are part of a white blood cell type called T-cells or T-lymphocytes. They protect the body from infection and stimulate the immune system after detecting any viruses, bacterias, or other intruders.
Q: What is the CD4 count of a healthy adult?
A: 500-1,200 cells/mm³.
Q: What do the test results mean?
A: A progressive decrease in CD4 count below 200-250 cells/mm³ indicates that the patient is HIV positive. Less than 200 cells/mm³ – pneumocystis pneumonia (PCP) and less than 50 cells/mm³ – mycobacterium avium complex (MAC).
Q: How often is the test ordered?
A: After being first diagnosed with HIV for baseline measurement, 3-6 months during treatment, and 6-12 months after CD4 levels start to increase.
