Prostate specific antigen (PSA) is a protein that maintains the semen in a liquid state so that the sperms can move freely. It is often present in blood in very low amounts. PSA levels are elevated in cases of prostate cancer or any other prostate disorders.

PSA test is often and most commonly done to screen for prostate cancer. However, this test is also performed to monitor progression of the cancer, see if the treatments are working, or if the cancer has come back. PSA test is also recommended when any abnormality in the prostate gland is noticed during a physical examination or other tests suggest towards the presence of prostate cancer.

PSA test is done using a blood sample. A small puncture is made in a vein in the arm to collect the blood sample. The samples are sent to a laboratory and results usually take a few days. Normal PSA levels are in the range of upto 4 nanogram per mililitre. In men of age group 50 and younger, PSA levels should be below 2.5 ng/mL.

PSA test can help detect several issues other than prostate cancer. A rise in the PSA levels can be due to benign prostatic hyperplasia (BPH) or prostate enlargement, prostate infection, UTIs, due to certain tests such as cystoscopy or biopsy, catheterization procedures, etc.

Certain medications, such as statins and non-steroidal anti-inflammatory drugs, can interfere with the PSA levels. Obesity has shown to reduce PSA levels. PSA levels also increase with age. Levels of PSA above 4 ng/mL is considered to be at a risk for prostate disorders. Levels above 4 ng/mL upto 10 ng/mL increases the risk of prostate cancer by 25%. Levels above 10 ng/mL are dangerous and are associated with a 50% risk of prostate cancer and require immediate medical advise.

High PSA levels require additional testing like a digital rectal examination or a prostate biopsy to confirm the absence or presence of prostate cancer. In some cases, prostate cancer is present even when the PSA levels are not elevated, giving false negative results. Hence, additional tests like urinalysis, urodynamic tests, transrectal ultrasound, prostate biopsy, and imaging tests like MRI or CT scans are required to confirm the diagnosis. False positive results are obtained when PSA levels are high due to certain conditions such as infection, irritation, BPH, etc.

No extra preparation is required for this test. However, ejaculation interferes with PSA levels and a patient may be advised against it for a few days before the test. It is important to let your healthcare provider know if you are undergoing treatment for BPH, prostatitis, etc. as the medications can lower PSA levels.

There are no major risks associated with this test. Minor risk may involve bleeding, multiple punctures to locate veins, fainting, and infection. These issues resolve within a few hours or days.
