C Peptide or Connecting Peptide is produced in pancreas during the insulin production phase. Pancreas first generates Proinsulin that splits to produce C peptide and insulin. As C peptide and insulin are produced simultaneously, C peptide testing serves as a marker for insulin production.
The C Peptide test is not ordered to know if a person is diabetic or not. However, the test serves to distinguish between type 1 and type 2 diabetes. Besides, people under insulin therapy start developing antibodies against it after a while. This makes it difficult to diagnose body's production of insulin. In such cases, C peptide test can be a helpful alternative. C peptide levels suggest a similar production of insulin within body. Moreover, C peptide test is beneficial in determining pancreatic tumor.

Patients showing symptoms of diabetes or hypoglycemia; such as, sweating, excessive hunger, nervousness, seizures etc. are often suggested to undergo a C peptide test.
C peptide testing does require some form of preparation. Most patients are asked to fast for around 8 to 12 hours prior to sample collection. Talking to your doctor regarding fasting hours is suggested. The procedure is however simple and involves collection of small amount of blood from your arms.
People who are taking blood thinners need to inform their doctors before sample collection. At rarest of cases, the procedure may result in bruises or other discomfort. Therefore, getting the test done from a recognized and experienced practitioner is always recommended.
Patients who are under constant insulin medication cannot have accurate C peptide evaluation. Individuals who do not have a functional kidney cannot undergo this test. Kidneys are responsible in eliminating insulin and C peptide from body, and thus maintain a balance. Therefore, kidney failures result in excess C peptide in body even when pancreatic functions are normal.
Obesity may also interfere accurate C peptide evaluation within body. Individuals who are obese, produce large amounts of insulin and thus C peptide. Therefore, they are very likely to produce some erroneous results.
