MRI Spectroscopy also known as Magnetic Resonance Spectroscopy (MRS) is a type of investigation which involves studying a particular body tissue for any metabolic changes that may have taken place. It is similar to MRI but with slightly advanced techniques. MRS is used for the study of nervous system most of the times.

Metabolism is a normal physiological phenomenon taking place in all body tissues. It leads to the formation of certain byproducts which could remain deposited in the tissue. MRS analyses the presence and quantity of these metabolites in order to come to a conclusion.

It cannot be a primary investigation in any patient. Diagnosis is usually done with the help of MRI scan, CT scan, USG etc. MRS helps in studying the abnormal tissue even more clearly to determine the intensity of damage.

Who performs MRI Spectroscopy?

MRS is performed by a team qualified radiology technicians headed by an experienced neuro-radiologist. It is a highly advanced procedure and requires great precision and skill. Therefore, it is essential that the person who performs this procedure should be well versed and experienced with the details.

Where is MRI Spectroscopy performed?

MRS is performed in the radiology department of hospitals or in a diagnostic center. The place where MRS is performed should we equipped with all the technology and machinery required to conduct this procedure and interpret its results.

Indications of MRS:

As mentioned earlier, in most cases, MRS is used for the study of central nervous system. It is usually done along with MRI to enhance the accuracy of diagnosis. MRS helps in assessing tissue damage in the following conditions:

  • Epilepsy
  • Senile dementia
  • Alzheimer’s disease
  • Degenerative brain disorders
  • Brain tumors like glioma, meningioma
  • Spine tumor
  • Degenerative spine disorders
  • Brain tissue damage resulting from exposure to radiation

Pre-Requisities for MRI Spectroscopy procedure

MRI Spectroscopy requires some amount of pre-planning before it is performed. It is not performed without a doctor’s prescription. Therefore, the doctor has to ascertain a few things before prescribing MRI spectroscopy. Some preparatory steps for MRI Spectroscopy have been given below:

The patient has to undergo a physical examination before being eligible to undergo MRS. The vitals including blood pressure, pulse rate, body temperature are monitored.

Patient’s previous medical reports are reviewed to find more about his/her health history. The patient has to notify doctor about any current health condition they might be having.

The doctor should know about the patient’s medication schedule. It is possible that some of the medications taken might be stopped or their dose might be altered for a few days before and after MRS study.

Female patients have to inform their doctor if they are pregnant or planning a pregnancy in immediate future.

Patients with any kind of implanted device like coronary stent, chemoport device, ICD, pacemaker, shunt or artificial prosthesis, artificial joint, orthopedic nails and screws etc., should inform the doctor about it.

Doctor should be made aware if the patient has any past or currently active kidney disorder.

How is MRI Spectroscopy performed?

Procedure of performing MRS is essentially similar to Magnetic Resonance Imaging, but it is more detailed. The procedure has been explained below:

Patient must arrive well in advance of the designated time to undergo MRS. The patient is asked to change into a surgical gown which is clean and sterilized. Any metallic jewellery or accessories need to removed before the procedure.
The set up is just like how it is needed for performing an MRI scan. In addition to the magnetic field and a sliding bed required for MRI, a device known as coil is used for MRS.

Coil is attached to the part of body which is to be specifically studied. Coil is designed in such a way that the body part to which it is attached is studied in more detail and clearer images are obtained.

The patient needs to lie down on a sliding bed in an appropriate position so that the affected body part can be studied well. The sliding bed is then exposed to magnetic field of a predetermined intensity.

The field is concentrated more on the area where coil is attached. Additional tests are performed along with exposure to magnetic waves. These tests help in aligning the hydrogen atoms within body tissues in a particular way.

The magnetic waves and change in alignment of hydrogen atoms helps in creating a series of images of the body part which is being studied.

Sometimes, a contrast dye is injected into the patient’s veins before the test begins. This dye circulates throughout the body and is absorbed by the affected body tissues as well. The clarity of MRS results is enhanced even more by this method. However, patients with kidney disorder cannot be given a contrast dye.

As mentioned earlier in the article, MRS studies detect the levels of metabolites in a particular body tissue to determine the level of damage which could have occurred. It also helps in classifying types of brain tumors and differentiating between brain tumor and brain tissue necrosis.

The procedure takes a bit longer than a plain MRI scan. It can last for over an hour, as the diseased tissue has to be studied in more detail.

Post-procedure steps:

After MRS study has been completed, the patient is discharged to go home soon. If the patient is experiencing any discomfort, he/she could be retained for sometime to be kept under medical observation. Due to the contrast dye, patient may experience transient nausea and vomiting, which subsides on its own.

Patients who have received a contrast dye injection, should take utmost care to maintain good hygiene. The dye gets excreted through urine for 3-4 days after it has been injected. Patients should ensure that they maintain high standards of toilet hygiene. They should wash their hands before and after handling day to day objects like towels, napkins etc. Family members and medical care providers of these patients should take care to avoid all contact with the patient’s body fluids.

Advantages and Disadvantages of MRI Spectroscopy:

Advantages:

MRS is a fairly advanced procedure and very helpful in determining minor abnormal changes and differentiating between types of tissue damage. It does not employ any harmful radiation like X-rays in order to determine tissue abnormality. The study is highly advanced and gives superior results than simple MRI. This is very helpful in determining the case prognosis and course of treatment.

Disadvantages:

Like any medical procedure, MRS has its own share of drawbacks. The process is quite time consuming and may not yield any extra-ordinary result every time. Since magnetic resonance is used for the procedure, it is inadvisable for women to undergo this test in the first trimester of their pregnancy. The use of contrast dye is contraindicated in patients with active kidney or blood disorders.

With adequate precaution, the disadvantages associated with MRS can be avoided as well. The procedure results play a major role in determining further treatment for the patient. To ensure proper results, it is important for the patient to co-ordinate with their doctor and provide full co-operation before, during and after the procedure. Patients must follow all instruction provided regarding MRS and heed their doctor’s advise fully. A successful outcome of the procedures goes a long way in prompt and successful treatment of the patient’s condition.
