Synovial fluid analysis is a diagnostic procedure which helps in detecting pathological changes occurring within any joints of the body. This procedure requires for the patient to visit a hospital or medical center equipped with a well-equipped radiology and pathology lab.
The test is always conducted in the presence of an orthopedic surgeon and pathologist. The technique involved in aspirating synovial fluid from affected joints is complicated and requires thorough knowledge of the anatomy of human bones and joints.

Every joint of the human body is formed by 2 or more bones. Easy movement of the bones to perform various movements is possible due to lubrication of the joint surface by synovial fluid. However, at times the amount of synovial fluid may increase due to underlying pathological changes within the joint. This fluid built up is unhealthy and requires immediate medical attention. A synovial fluid analysis test could be advised in such cases.
Indications of undergoing synovial fluid test:
- Swelling of joints
- Pain and redness of joints
- Severely restricted joint movement
The affected joint is kept well exposed in order to collect synovial fluid. A large bore needle is inserted into the joint under USG, MRI or CT guidance. This prevents injury to neighboring healthy tissue. Fluid accumulated in the synovial space is aspirated to be used for testing. The fluid is collected in sterilized glass bottles and kept away from heat, light and moisture.
Synovial fluid is of physiological importance as it helps in lubricating the joints and helps in their free movement. Therefore a minimal amount of fluid has to be retained within the joint. This should be remembered by the technician or doctor who is performing synovial fluid collection. The process of collecting synovial fluid for testing has a direct therapeutic effect on the patient’s health. It helps in reducing the abnormal built up of fluid within the joint, thus providing some relief to patients.
Synovial fluid analysis includes macroscopic as well as microscopic examination. Appearance, color, viscosity of the synovial fluid are important factors for diagnosing internal health of joints. Microscopic examination of synovial fluid is performed to detect presence of red blood cells, white cells, pus cells, cellular debris etc.
Along with this test, patients may also be asked to undergo X-ray or other scans of the affected joint.
