Symptoms of inflammatory disorders such as severe headaches, sudden weight-loss, reduced diet, stiffened and paining joints, prompt the doctor to order an ESR for his patient. An ESR helps the doctor in determining if his patient has inflammatory disorders or some other underlying medical condition like cancer. It can also be ordered by the doctor in order to keep a tab on any of the inflammatory condition and autoimmune disorder already present. This enables the doctor to also track the patient's response towards treatments for the same.

No specific preparation is required before the test. You should discuss any medications you are currently on, as specific medications have the capability to interfere with results.

The ESR test is done through basic blood testing. It does not take more than 5 minutes to draw a sample from the arms of the patient. The blood sample is allowed to sit in a test-tube for over 60 minutes. Behavior of Red Blood Cells in the blood is mapped and observed. The speed and number of RBCs sinking down to the bottom is used for calculation of results. Hereby given is range of normal ESR values - Men: <15 mm/hr (under 50 yrs) and <20 mm/hr (over 50 yrs). Female: <20 mm/hr (under 50 yrs) and <30 mm/hr (over 50 yrs). Newborn: 0-2 mm/hr and 3-13 mm/hr (until puberty).

High values of ESR may be because of autoimmune diseases, rheumatoid arthritis, high cholesterol, kidney diseases, multiple myeloma, old age and even pregnancy. Tuberculosis, skin and bone infections, heart diseases are also responsible for ESR values that are higher than normal. Whereas, lower ESR values are because of congestive heart failure, sickle cell anemia, low plasma protein, or leukocytosis. The RBCs in a person who has serious inflammation, reach the bottom of the tube at a faster level.

ESR is rarely ordered as a single test. It is ordered in conjunction with Complete Blood Count (CBC) and Polychlorinated biphenyls (PCBs).
